In latest patch 5.1 Nurgle settlements have been updated so that 2 population surplus is required for upgrading settlements to tier 3 instead of 3 population surplus prior to the patch. This patch has applied to minor settlements within a province but the capital province still requires 3 population surplus to upgrade from tier 2 to tier 3.
Step by step of the bug
Step 1
Load campaign as Nurgle (tested with Kugath and Epidemius)
Step 2
Occupy any capital settlement within a province
Step 3
Upgrade capital settlement to tier 2
Step 4
Tier 3 upgrade for capital settlement requires 3 population surplus instead of 2
